Identify. Understand. Act. Embed
Everyone should have a fair opportunity to access high-quality cancer care, regardless of who they are, where they live, or the circumstances they face.
The SWAG Cancer Alliance Health Inequalities Framework provides a practical approach to identifying and addressing unfair and avoidable differences in cancer prevention, diagnosis, treatment, experience and outcomes. It supports teams to understand who may be missing out, explore why inequalities exist, and take meaningful action to improve cancer care across Somerset, Wiltshire, Avon and Gloucestershire. Through a combination of population health data, community insight and partnership working, the Framework helps make health equity a routine part of how we design, deliver and improve services.

Built around four key principles, the framework helps teams recognise underserved populations, understand barriers, identify evidence-based solutions and embed equity into routine practice.
The Framework is supported by a dedicated Health Inequalities Working Group, which includes at least one nominated Health Inequalities Link (HIL) from each cancer programme. The group provides a forum for challenge, shared learning, collaboration and oversight, helping to ensure that health inequalities remain a consistent focus across all areas of work.
The SWAG Cancer Alliance Health Inequalities Framework will continue to evolve through ongoing collaboration, learning and feedback, ensuring it remains relevant, practical and responsive to the needs of the diverse communities we serve across Somerset, Wiltshire, Avon and Gloucestershire.
Work With Us
Reducing cancer health inequalities cannot be achieved by one organisation alone. We want the Framework to be shaped and influenced by the communities we serve, NHS organisations, local authorities, Voluntary, Community, Faith and Social Enterprise (VCFSE) partners and people with lived experience. We recognise that reducing inequalities requires shared decision-making, shared learning and shared accountability.
We're looking to connect with organisations and individuals who share our commitment to improving cancer outcomes and experiences for underserved communities. If you would be interested in learning more about the framework or contributing your expertise, we would love to work together. Please do get in touch to:
- Discuss opportunities to work together to reduce health inequalities
- Learn more about the SWAG Health Inequalities Framework and how we are using it to guide our work
- Share your views and help strengthen our approach
- Show your support for the Framework and our shared commitment to reducing inequalities
- Join a growing network of partners who want to stay connected, share learning and work together to make a difference
Whether you would like to explore a potential partnership, find out more about the Framework, or endorse this approach, we would love to hear from you.
