Continence Specialist Nursing Team
The continence specialist nursing team supports patients with bladder and bowel problems and promotes healthy living. They offer assessment, support and advice to patients who have continence needs.
The team provides a service to all GP practices within South Gloucestershire. There is also an inpatient dietetic service provided to Thornbury Hospital.

The Creative Arts team provide a specialist creative keepsake service that enables patients and their families to have time together to creative a lasting memory of the person and family. The team support a number of bereaved families to come to terms with the loss of their loved ones.
The team also provide support to day patient and inpatients unit in the form of creative activities that provide some meaningful activity to the person.
A facilitated art group is in place to provide a safe and supported environment for patients to explore their feelings in a creative way.

Community Connect is a service for people over 50 living in North Somerset who would like information, advice or guidance to help them stay living independently at home.
It also gives people the chance to offer their time and skills at events and groups in local communities.
Community Connect can offer you a wide range of support and advice that’s tailored to you.
We can provide:
Information and guidance over the phone, face-to-face or by email
Tailored support for people over 50 to stay connected and active in their communities
Help for you and your community to set up groups and activities that benefit the local area
Opportunities to participate and learn a new skill

Daisy Network was created to provide support to women, along with their families and partners, who have been diagnosed with Premature Ovrian Insufficiency (POI). We understand that this diagnosis can feel incredibly isolating and often women are left confused and unsure where to go next.
Daisy Network provides our members with information and the latest research findings on various aspects of POI including:
– Hormone Replacement Therapy (HRT) and other treatment options
– Managing the longer term health implications such as bone and cardiovascular health, nutrition, and the psychological impact
– Egg donation, adoption or leading a positively childless life

Rebekah sees all other oncology and haematology patients under an RUH consultant. Her post is not full time for oncology so has limited capacity

The Macmillan dietetic clinic offers the opportunity for people to self-refer, or to be professionally referred for 1:1 nutritional guidance from a registered Macmillan Specialist Dietitian. Referrals are accepted at any stage following a cancer diagnosis. Support options include; face to face appointments, video calls and telephone consultations.
‘ACT on Weight’ course also available for combined dietetic and psychology support for weight management. Appropriateness assessed in Macmillan clinic above.
Separate MDT clinics are available to support patients with upper gastrointestinal cancers.

This new service has been set up to develop outpatient services for patients who have a nutritional need as a result of their cancer or cancer treatment.
The Nutrition in Cancer clinic offers one to one appointments. The service also inputs at Next Steps health & wellbeing clinics and is planning to provide a similar service for pre-treatment and palliative patients

If you’ve been given a code or codes by your GP surgery to help you access online services, please also bring this with you. If you don’t have a code or aren’t sure why would need one, don’t worry – we can still support you on the day.

Aged 18 to 24End of Life Care South Gloucester
This service offers an End of Life Coordination Centre, supporting adults over 18 with a life limiting illness, registered with a South Gloucestershire GP and who are in the last few weeks or days of life.
The Centre acts as a link between professionals, carers and service users, ensuring that timely packages of care are arranged and meet the needs of the service user, carer and families.
Care may be provided from the following services:
Domiciliary care agencies
Palliative Care Home Support (PCHS)
Marie Curie Support Workers
St Peter’s Hospice at Home Service
The Co-ordination Centre can also access overnight care for people at home (Night Sitters). This may be used to give carers a break for a night or two, for example. This can often help carers to continue to support their loved one at home.
The End of Life Co-ordination Centre will also act as a link between professionals, service users and families. For example, this may consist of ringing service users or carers to let them know what care has been arranged, to check if the right amount of care is being provided or assisting in providing equipment to support the person and carer.

Exercise programme for people living with or recovering from cancer.
Energise Bristol offers cancer patients a structured introduction to exercise in a safe, supported way. Patients can attend before, during or after treatment.
They are offered individualised assessments before starting the 12 session programme which consist of gym and circuit style exercise. The programmes are run by Level 4 Can Rehab Exercise Instructors
Discounted leisure centre membership is offered on completion of the schemes
Patients can self-refer or be referred by a healthcare professional
